Service Description

To ensure that your Physical Therapy (PT) evaluation is covered by your insurance, please provide the following: - a copy of your insurance card - date of Birth - a physician’s referral This will help verify if both your evaluation and treatment are covered by your insurance plan. Please provide the required information at least 72 hours before your appointment to allow sufficient time for verification and to ensure a smooth scheduling process.
